Information For the Michigan Better Kid Care On-Line Training on Sudden Infant Death Syndrome and Safe Sleep

You are being invited to participate in an evaluation of an on-line training program for childcare providers, parents and other caregivers in Michigan. The title of this on-line training program is Sudden Infant Death Syndrome and Safe Sleep. The purpose of this research study is to find out how well the training program is preparing participants to understand and prevent SIDS through safe sleep practices. In addition, we want to better understand how participants feel about on-line learning and improve the on-line training program for future participants.

After you register and view the power point and read the handouts provided on the website, you will be asked to complete two on-line questionnaire. These questionnaires contains questions about yourself, what you learned in the training, your interest in on-line learning opportunities, and your attitudes toward using computers.

Your participation in this evaluation is voluntary. You have the right to refuse to answer any question, or even refuse to participate at any time, without penalty. You are still able to view the power point presentation but we will be unable to give you a certificate of completion if you do not complete the questionnaires. There are no known risks associated with participation in this evaluation. You will not directly benefit from participating in this evaluation, however, your participation will lead to a greater understanding of the topics and needs of child care providers in Michigan and will lead to changes and improvements in child care provider training.

The information you provide will be kept confidential and only shared with the principal investigator, Dr. Dawn Contreras , and co-investigator Kendra Moyses . Your name will not be attached to any of your individual responses when reporting results of the research study. The information you provide on the questionnaires will be kept on a password protected computer. Your privacy will be protected to the maximum extent allowable by law.
